Even though, we are in the era of EHRs (Electronic Health Records),
a personal health journal captures various snapshots of your health, which helps you and your doctor get a clear picture of your medical history. The American Academy of Family Physicians mentions these added benefits of keeping a health journal: 1. You can track your illnesses, as well as the various treatments you've taken. 2. You can track medications you've taken, and any surgical procedures. 3. You can take your journal to doctor's appointments and review with your physician. 4. You can become a more empowered, knowledgeable and informed patient.
